Client History: Symptoms

Access the Client History window from the client’s record or the client intake window.

Symptoms

Indicate the client’s rating of the severity of any symptoms experienced by the client.

Be sure to use the client’s rating of each stressor and symptom group in the history. The clinician’s assessment (rating) of the stressors and symptoms is available in the Client Assessment window.

Indicate if the client is experiencing symptoms of:

Depression

Behavior/Anger/Impulse Control

Bipolar Disorder

Anxiety

Somatic Complaints

Self-Harmful Behavior

Psychotic/Thought Disorder

Sleep Problems

Memory Loss or Disorientation

Obsessions/Compulsions

Dissociative State

Neurodevelopmental Issues

Personality Disorder

Eating or Nutritional Issues

Gender/Orientation Issues

Grief/Loss

Harmful Behavior (Others)

Phobias or Excessive Fears

Low Self-Esteem

Ability to Care for Self

Elimination Disorder

Sexual Difficulties

Paraphilic Tendencies

As the history is completed, the clinician can indicate the severity of any stressors or symptoms within some sections. The clinician's assessment of severity can also be completed or edited in the client assessment.
