Keep your client records up to date

Progress Notes

Pathways automatically creates a progress note for each client session. The point-and-click format helps you quickly complete the note, making it easy for you to stay current on your paperwork requirements. Assess the client's progress toward treatment plan objectives, documenting the methods used to work towards achievement of each objective. As objectives and methods are achieved, update the client's treatment plan directly from the progress note.

Cancelled Sessions

Pathways 7 includes notes for cancelled sessions. If a client fails to attend a scheduled session, complete the optional note, and send it to the client's caseworker or other contact if desired.

Progress Reports

Sometimes actual progress notes of each session are not appropriate to send to a caseworker or other authorized agency. A Progress Summary allows you to easily report the client’s progress in therapy without including session notes.

Categories include topics of sessions, client progress towards treatment goals, ongoing areas of focus or concern, and recommendations.

 

Integrated System

Pathways 7 includes expanded and integrated client history and assessment features. The client history window follows the format and content of the included client history form, and allows the clinician to assess the severity of issues as related by the client. The Pathways assessment includes mental status, client strengths/weaknesses, identified problem areas, DSM-5 diagnoses and planned treatment. The client history and assessment integrate with the client treatment plan, so there's no duplicate entry of information. Problem areas that are noted during the client history or assessment can be easily added to the client's treatment plan.

Progress Notes: General

The expanded client history links to the client assessment and treatment plan, so client needs and goals can be quickly identified and documented. The client history window follows the format and content of the included client history form, and allows the clinician to assess the severity of issues as related by the client. As details are completed, the clinician can indicate if psychosocial events impact or are impacted by mental health issues. If they are, Pathways will automatically trigger an Action Point. Action Points are used to suggest goals and objectives for the client's treatment plan.
