Click the Add New button to add objectives or methods to the client's treatment plan.






If an objective or method is marked as complete or achieved, you will be prompted to update the client’s treatment plan. With Pathways’ integrated features, this can be done directly from the progress notes window. Read More >>



From any client’s record, click “Progress Reports.” In the window that opens, all progress reports that have been created to date for that client will be displayed on the right. Simply select the date of the report you wish to view.

If you have not created a progress report yet, complete the report title, the date of the report, and the period to which the report applies. To create a new progress report, click the Add New button. Enter the date of the report and the period for which the report applies. When you’ve done this, click Review to enter information in the progress report.

Problem Addressed

Select a problem from list to view current objectives for the problem.

Always add the primary problem addressed during the session first; the associated diagnosis for the appointment will be linked to the first problem entered for the progress note.


Select the appropriate diagnosis for the session. If there is no diagnosis associated with the current problem, click the arrow button to view client diagnoses.

Objective(s) Addressed

Once a problem has been selected, Pathways will list all current objectives for the client that apply to that problem. Select an appropriate objective that was addressed during the session. If you have not created a treatment plan, you can type in objectives for the progress note, but they won’t be linked to a treatment plan. Methods cannot be typed in.

Associated Methods

For the selected objective, select an associated method that was used or addressed in the session.

Therapeutic Intervention

Select the appropriate intervention from the options, or select “Other” and type in a description.

Client Participation/Action

Indicate the client’s action or participation in work toward achieving the indicated objective.

Client Progress

From the list, indicate the client’s progress toward achievement of the indicated objective: None, Minimal, Moderate, Considerable, or Completed. This list is not editable.


Enter any remarks or notes pertaining to the selected objective and the client’s participation or progress toward achieving it.

Once the information pertaining to the objective has been completed, use the Next arrow to add another objective to the progress note, if desired.

Clinical Impression/Notes

For any areas which were not described in another portion of the note, provide a narrative here.


Type, or select the appropriate plan from the dropdown list. Right-click to edit the list of available plans.
