Client Assessment: Problem Areas

Problem areas are a combination of client stressors and reported symptoms. In the client assessment, the clinician assigns a rating to each area, and indicates if it will be a target of treatment.

In this section of the client assessment, problem areas are sorted by client stressors, then symptoms. Use the scroll bar on the right to view all problem areas.

Severity (Client)

The client's rating of each stressor and symptom group, as indicated in the client history, are displayed and can be edited in this window.

Severity Level (Rated by Clinician)

If the clinician's assessment of a stressor or symptom was completed in the client history, it will be automatically displayed in the client assessment.

Assess the severity of each problem area using the following scale:

None: No symptoms or problems are apparent or reported.

Mild: Slight indication of the issue, with little or no impact. Generally not a target of treatment.

Moderate: Indications or symptoms of the issue are apparent, and are significant but not overwhelming.

Severe: Strong indication of issue or symptoms, causing significant impairment in daily functioning.

Extreme: Overwhelming indications of issue or symptoms requiring immediate action or intervention.

Target of Treatment

Indicate if the problem area will be a target of treatment. The default for each problem area is "no." If you are not sure that the problem area will be an immediate target of treatment, check "deferred."
